- Passport (and visa if required)
- Driving licence (if applicable)
- All relevant training certificates
- Most recent DBS certificate
- Two proof of address (dated within the last three months)
- Bank details
- National Insurance Number
- Next of Kin details
- Five years address history
- Five years’ work history
